Grievance Procedure

The Family Center of Colorado Springs, PC is fully committed to conducting all activities in strict conformance with the best practices of mental health. The Family Center of Colorado Springs will comply with all legal and ethical responsibilities to be non-discriminatory in promotional activities, program content and in the treatment of program participants. The monitoring and assessment of compliance with these standards will be the responsibility of the Privacy Officer, Gayle Christensen or other direct supervisors.

While The Family Center of Colorado Springs goes to great lengths to assure fair and ethical treatment for all participants, there will be occasional issues that require intervention and/or action on the part of management of The Family Center of Colorado Springs. The list below outlines additional options for filing such grievances.

  1. If you believe we have violated your privacy rights, you have the right to file a complaint in writing with our Privacy Officer, Gayle Christensen. Submit your complaint at 1751 South 8th Street, Colorado Springs, CO 80905 with it marked ‘Attn: Gayle Christensen’ OR at gayletfc@gmail.com.

  2. A complaint/grievance may also be submitted to the Behavioral Health Administration of Colorado by submitting the BHA Individual Complaint Form at https://bha.colorado.gov/contact/contact-us, emailing CDHS_BHA_complaint@state.co.us, or calling (303) 866-7191

  3. A complaint/grievance may also be submitted to the Colorado Department of Regulatory Agencies (DORA) at https://dora.colorado.gov/file-complaint

  4. To file a complaint with the Secretary of Health and Human Services at 200 Independent Avenue, S.W. Washington, D.C. 20201 or by calling (202) 619-0257.

    You are within your rights to file a legitimate complaint and we will not retaliate against you for filing a complaint.
